Prospective patients can either complete their Yellow Medical Card or simply request a new patient registration form from reception. Patients will be registered with the Practice and not with a specific doctor. Medical treatment is available from the date of registration. New babies should be registered with the practice as soon as possible after leaving hospital, please bring along your new born child’s birth certificate with you.
